Pursuant to Local Rule 83-5, attorneys who have been admitted to practice by the Supreme Court of the United States or the highest Court of any state may become a member of the bar of this Court. Apply for permanent admission to the Southern District of Indiana by submitting an application form with an original signature and the fee of $231 to the address noted below. Checks should be made payable to "Clerk, U.S. District Court." Bar admission applications will be processed in the order received, and as expeditiously as possible. Please allow five (5) business days for processing.

Pro Hac Vice:
Pursuant to Local Rule 83-6, attorneys may also be admitted to appear in a particular case by filing a Motion to Appear Pro Hac Vice, along with a proposed order, and a $100 fee per attorney, per case.

- Sample 2 - Motion/Order to Appear Pro Hac Vice: (Please use use this form if you are moving for the PHV admission of another attorney -- not admitted to the U. S. District Court for the Southern District of Indiana.)
NOTE: Electronic filing is required in the Southern District of Indiana. When appearing pro hac vice, please submit an ECF Registration Form and review the court's E-Filing Resources for more information.
Certificate of Good Standing:
An attorney's "Certificate of Good Standing" and, if needed, a "Statement of Discipline" may be requested through the Clerk's Office. Please mail or fax the request along with an $19 fee. Checks should be payable to "Clerk, U.S. District Court."
