Congratulations
and welcome to the second stage of the jury process! You have
received a Notice of Jury Service because you were qualified to
serve in Stage 1 and your name was randomly selected from the list
of qualified jurors. Your jury package should include a
Notice of Jury
Service
questionnaire, a Federal Jury Service brochure, and a postage-paid
envelope. The purpose of this Notice of Jury Service is to provide
you with the dates of your one-month term of service and to collect
updated and supplemental information. Simply complete the Notice of
Jury Service questionnaire and return it in the enclosed,
postage-paid envelope. The start of your one month term of service
is indicated on the Notice of Jury Service and enclosed brochure.
You are considered “on-call” during that one month period, which
means the court may issue a Summons to Appear with specific
reporting instructions.
During Stage 2, access to your jury status is available 24 hours a
day, 7 days a week, by using the court's Automated Jury Information
System. Simply dial 1-877-803-2073 and enter your participant
number. You can obtain information specific to your status,
reporting instructions, excuse requests, and court contact
information.

Provide your date of birth and contact information.

Provide your new address if you have moved.

Provide any dates you are unavailable to serve during your
one month “on call” term of service, such as a scheduled
vacation, doctors’ appointments, etc., that cannot be
changed. The court will honor all reasonable
requests.
